If your Practice Permit expires on March 31st, 2012 please send your Professional Development Credit Form (PDCF) in to us by February 29th, 2012. Please do not send in supporting documentation with your PDCF. You may be randomly selected to send in a portfolio by November 30th, 2012, and you will receive notification of this by mail in June. At this time, you will send in all of your portfolio documents, including your supporting documents.
